The case involved Mr. Doe, a patient who developed severe pressure ulcers while under the care of a Skilled Nursing Facility (SNF). Despite being under professional care, his condition worsened significantly due to apparent lapses in the standard of care. Over a period from 06/21/YYYY to 07/27/YYYY, he experienced a deterioration in skin condition, worsening ulcers, and overall health decline. 

Issues in the Case 

Mr. Doe’s Attorney sought Trivent Legal’s expertise to develop a clear, concise chronology of the events surrounding the development and worsening of the pressure ulcers. The attorney needed to demonstrate the direct correlation between the lapses in care at the SNF and Mr. Doe’s deteriorating condition.  

Given the massive volume of medical records and the short preparation time, the attorney required a streamlined analysis that highlighted critical deviations in care without overwhelming detail.

  • Identification of Deviations in Care: Our analysis revealed substantial deviations in the care provided by the SNF, including delays in wound consultations, inadequate assistance with ADLs, and neglect in repositioning practices. These deviations were directly linked to the patient’s deteriorating condition and the worsening of their pressure ulcers. 
  • Simplified Presentation for Client Understanding: To accommodate the client’s limited time, Trivent condensed the 11,207 pages of medical records into a concise 7-page document. This document focused on the key findings and discrepancies in care, enabling the attorney to quickly grasp the critical issues in the case and prepare an effective legal strategy.
